Health centers have a critical role in patient and community education, outreach, and health navigation/coordination. There are a multitude of current and upcoming opportunities for health centers to expand or enhance services providing community-based linkages. Michigan health center staff have the knowledge and skills needed to help patients and communities address barriers and social determinants of health. Michigan Primary Care Association (MPCA), in turn, assists health center staff through technical assistance, training, peer communication network facilitation, resource development, and best practices sharing.

The Enabling Services Annual Forum: Building a Bridge to Health Care

On October 4th and October 5th, outreach and enrollment specialists, navigators, and community health workers from across Michigan joined the MPCA for our annual Enabling Services Forum: Building a Bridge to Health Care. The first day of the conference included a training session on motivational interviewing presented by MPCA’s Director of Integrated Health and Programs, Sara Coates. Motivational interviewing is a counseling approach that attempts to help a patient or client find the motivation to make positive decisions and accomplish established goals. The process is designed to be empathetic, affirming, and respectful: no goal is too small when it comes to making healthful changes. The second day of the conference boasted a host of panels, including an update from the Michigan Department of Health and Human Services (MDHHS) on how they’re streamlining and modernizing the MI Bridges experience, a session on navigating immigration status, an advocacy update, and much more. Financial Empowerment

 

On August 22nd, Community Economic Development Association of Michigan (CEDAM) and Step Forward Michigan discussed how to help low-income and economically vulnerable consumers to make informed financial decisions by providing them with the necessary tools and information. CEDAM supports its diverse membership to create vibrant, sustainable and resilient communities by enhancing locally-driven efforts through high quality training, technical assistance, capacity building and policy advocacy. Step Forward Michigan Program was established by the Michigan Homeowner Assistance Non-Profit Housing Corporation (MHA) to help Michigan homeowners stay in their homes. The program provides interest-free loans to assist with mortgage, property taxes, and/or condominium association fees. You can now access the webinar recording and updated version of the slides.

The MPCA Enabling Services team maintains two online toolkits aimed at providing health centers with policy information, resources, program planning tools, and templates needed to successfully launch and grow community health worker and outreach and enrollment programs.
